Well i am not being particular , take any psychological child disease. In a small age he is not mature enough to judge that he is suffering from some disease. The people with whom he interact are the only one who could judge his behavior.
parents really play a big role in this group. But what will happen if they don’t have time for their child, they are not with him during his happy or sad moments or , whenever he needs them. Scolding them, loving one child more than the other, putting them in boarding exacerbate the problem and a barrier starts building up and he stops sharing his feelings, his problems.  So when parents are out of picture school come as second option.
After home it is school where he spends most of his time. His friends at school, class mates, teachers could certainly tell whether some thing is wrong about him, his behavior.
